Description

Under general direction, the Program Manager leads the development and implementation of First 5 Alameda County's new Enhanced Care Management (ECM) program for young children and families with complex medical and social needs. This is a unique opportunity to build a program from the ground up that will improve access to coordinated, whole-person care for children and families across Alameda County. The Program Manager oversees program implementation, supervises staff, and ensures services align with CalAIM requirements, quality standards, and regulatory expectations. In partnership with clinical and community partners, the role develops workflows, protocols, and systems that support effective ECM service delivery while advancing child-centered care and birth equity.



Qualifications

Any combination of training and experience that would provide the required knowledge, skills, and abilities is qualifying. *Additional experience can be substituted to meet typical qualifications. A typical way to obtain the required qualifications would be:

5 years of experience in care management, case management, or health/social service delivery; 3 years of program management and supervisory, administration, or team leadership experience.

Master’s degree in social work or a related health field*
