Position Overview
This position is in the Chronic Disease and Injury Prevention Program of Austin Public Health in the Disease Prevention and Health Promotion Division and will supervise Austin Public Health’s Healthy Living Program. The Healthy Living Program empowers community members to prevent and manage type 2 diabetes through evidence-based, culturally appropriate community-based diabetes self-management and prevention classes, with a focus on uninsured individuals and communities experiencing health disparities. This position will supervise four Community Health Workers who provide the community-based diabetes education and prevention classes and provide quality assurance including management of program data, ongoing training on diabetes topics, and maintenance of the program’s Centers for Disease Control and Prevention and Association of Diabetes Care and Education Specialists ( ADCES ) certifications. This position will also be a working supervisor position that is actively involved in teaching the nutrition components of the classes in community-based settings and may provide individualized diabetes education for a subset of clients that need additional education and support.

Duties, Functions and Responsibilities
Essential duties and functions, pursuant to the Americans with Disabilities Act, may include the following. Other related duties may be assigned.
Conduct planning, development and evaluation of assigned program. Provide supervision of program implementation. Perform grant writing and management of grant budget. Collaborate with community based organizations. Promote program and provide program information to community. Provide educational and community awareness presentations.

* The minimum qualifications for the Public Health Program Supervisor position are: Graduation with a Bachelor’s degree from an accredited college or university with a Bachelor’s degree plus two (2) years of experience in a field related to public health/human services programming, planning or program management, one (1) year of which was in a lead or supervisory capacity. Master’s degree may substitute for one (1) year of the required work experience. Experience may substitute for education up to a maximum substitution of four (4) years.
