Job Location:

This position is domiciled Central Missouri Regional office located at 900 W Nifong Blvd Columbia, MO 65202. Will have the ability to work remotely a portion of the week upon successful completion of the initial onboarding period.

Role Description

This position will serve as a team lead and coach for their Support Coordination team. This position will provide guidance, training, and coaching to Support Coordinators while also providing Targeted Case Management Services to a caseload. This position will support the Support Coordination Supervisor to ensure the SC team is providing quality support coordination services and documentation.

Knowledge, skills & abilities

Demonstrates the knowledge, skills and abilities outlined in the Missouri Support Coordination Core Competencies. Ability to coach and mentor staff to meet the Missouri Support Coordination Core Competencies, State Plan Requirements, and Medicaid Waiver Assurances.

Typical Qualifications

Bachelor’s degree and 2 - 4 years of relevant experience.
