The Nurse Navigator will provide specialized support to children and youth with special healthcare needs (CYSHCN) and their families within the HCP program. This includes offering information and referrals, coordinating care, facilitating access to services, and helping families navigate the healthcare system. The Nurse Navigator will collaborate with healthcare providers, specialists, and community resources to ensure coordinated, comprehensive care that meets individual health needs and improves outcomes. The role also involves working with the Maternal and Child Health (MCH) Block Grant to align with state health objectives, and contribute to strategic measures that promote racial equity, community inclusion, and those which address the root causes of health disparities in Larimer County’s MCH population.

The typical schedule for this Limited Term position is Monday through Friday, 8:00 AM to 4:30 PM, with hybrid remote work options available. This role requires availability for in-person meetings, home visits, and occasional community outreach events, including outside of regular work hours or Health Department operating hours, as needed.

Responsibilities

What you'll be doing:

• Conducting home visits and phone calls to assess the physical, emotional, social, and environmental needs of clients and their families, and developing plans to meet these needs according to program guidelines.
• Assisting families in understanding complex medical diagnoses and treatment options, providing emotional support, and empowering them to make informed healthcare decisions.
• Providing education, resources, and referrals as appropriate.
• Identifying gaps and barriers in community systems for clients and families.
• Consulting, coordinating, and collaborating with community resources to connect families with appropriate services.
• Helping families navigate insurance and public health programs by ensuring they understand coverage options and eligibility requirements.
• Building community capacity and serving as a resource for other community agencies and Health Department staff, offering guidance in areas of specialty.

• Ability to build effective relationships with clients, families, and care providers to develop individualized care plans for each child.

Qualifications

• Bachelor's Degree, or equivalent combination of education and/or experience, in Nursing from an accredited college or university required. Related education or experience may be substituted on a year for year basis.
• Licensed to practice as a registered nurse (RN) in the State of Colorado required.
• One (1) year experience in providing health services as a Registered Nurse required.
• Possession of CPR and First Aid certifications by date of hire required.
• Possession of a valid Driver's License by date of hire required.
• One year (1) of experience in specific nursing program relevant to the role they will perform preferred.
