The Nurse Navigator will provide specialized support to children and youth with special healthcare needs (CYSHCN) and their families within the HCP program. This includes offering information and referrals, coordinating care, facilitating access to services, and helping families navigate the healthcare system. The Nurse Navigator will collaborate with healthcare providers, specialists, and community resources to ensure coordinated, comprehensive care that meets individual health needs and improves outcomes. The role also involves working with the Maternal and Child Health (MCH) Block Grant to align with state health objectives, and contribute to strategic measures that promote racial equity, community inclusion, and those which address the root causes of health disparities in Larimer County’s MCH population.

The typical schedule for this Limited Term position is Monday through Friday, 8:00 AM to 4:30 PM, with hybrid remote work options available. This role requires availability for in-person meetings, home visits, and occasional community outreach events, including outside of regular work hours or Health Department operating hours, as needed.
