The Department of Public Health (DPH) is recruiting for Public Health Nurses II (PHN II) who provide assessments of individuals, families and communities. Duties include case management, coordinating and assisting with access to care; developing treatment plans; providing consultation on medical, dental, and behavioral health assessments; acting as a nurse liaison with public and private agencies; providing assessments and referral for school district students; and conducting disease control measures. Vacancies exists throughout Family Health Services (FHS) and California Children's Services (CCS) sections in DPH.

Family Health Services

Family Health Services (FHS) section of DPH specializes in many dynamic public health nursing programs and includes Health Care Program for Children in Foster Care, Rx 4 Kids, Children Screening, Assessment, Referral and Treatment, Black Infant Health, and Maternal Child and Adolescent Health. FHS works in partnership with public and private agencies to provide nursing case management, care coordination and consultation.


California Children's Services


The California Children's Services (CCS) section of DPH is a statewide program that arranges, directs, and pays for medical care, equipment, and rehabilitation, when these services are authorized for children and young adults under 21 years of age, who have CCS-eligible medical conditions and whose families are unable to pay for all or part of their care. CCS determines eligibility and authorizes qualified professionals to treat the child's CCS eligible condition.

CONDITIONS OF EMPLOYMENT

Travel/Driver License: A valid California Class C Driver License is required to carry out the essential job-related functions of this position. Mileage reimbursement and proof of automobile liability insurance must be maintained.

Pre-Employment Process: Prior to appointment, applicants must pass a background investigation which includes verification of employment history and education, fingerprinting, physical exam, and drug screening

Desired Qualifications

CCS Section: Experience working with pediatric populations which includes enhanced care management experience, certificate or training and the ability to use social determinants of health in developing care plans and interventions is highly desirable.

Applicants fluent in American Sign Language, Arabic, Korean, Mandarin, Spanish and/or Vietnamese are strongly encouraged to apply. Additional compensation is associated with positions requiring the use of bilingual skills. Testing of bilingual skills may be required to receive additional compensation.

Candidates possessing computer skills in Microsoft applications are preferred.
