Position Description
Employees in the classification design and implement health education programs designed to increase awareness and knowledge for the improvement of health in a single, group, or community setting. Keeps up-to-date on changes in health care technology to keep health education program material current. Employees in this classification receive limited supervision and work is evaluated based on progress toward a set of goals by immediate supervisor. May supervise student assistants and other clerical personnel. Work is performed under usual clinic and office conditions and requires a great deal of patient contact and contact with community resource people. Some work is performed after usual office hours.

Major/Essential Functions
This position will support the TTUHSC SON Combest Wellness Center in Lubbock, Texas as a Care Manager. In this position employee will serve as a member of the interdisciplinary care team and is responsible for providing care management for clinic patients. Will work with a diverse patient population. Primary responsibilities in this role include:

Required Qualifications
Bachelor's degree from an approved accredited university in a field relevant to the work being conducted; one year of work experience demonstrating health education practices, techniques, and principles.
