DescriptionTHE DEPARTMENTAmbulatory Care is a system of primary and specialty care medical clinics located across Ventura County. Currently, Ambulatory Care clinics provide care for over 40,000 individuals with hypertension and has been selected as one of three sites for the Comparing Hypertension Remote Monitoring Evaluation Redesign (CHARMED) trial. In this role, you will support functions pertinent to the CHARMED trial within Ambulatory Care. Your work will be instrumental in advancing the Ventura County Health Care Agency's mission to set the standard in health care excellence, while also driving our Agency toward becoming a leader in blood pressure management.
THE POSITIONThis position is responsible for developing and implementing educational and recruitment materials to encourage patient participation in the CHARMED study. The role involves direct outreach and engagement with potential participants to ensure clear understanding of the study’s purpose and procedures. Additionally, this position will oversee data collection, ensure data integrity, and conduct analysis to generate reports and support research. This position will work closely with the Quality Improvement team and will also oversee a team of community outreach professionals.
THE IDEAL CANDIDATEThe ideal candidate will be a detail-oriented, proactive, and flexible individual, with an interest in working collaboratively with a research team on a variety of projects. The ideal candidate demonstrates excellent relationship-building, customer service, and public service skills. They will have strong communication and active listening abilities, effectively engaging with individuals and groups from diverse cultural, social and economic backgrounds. They will be compassionate, patient, and possess a genuine desire to assist others. Additionally, time management, planning, and organizational skills will be key. The ideal candidate will be comfortable meeting documentation standards. Finally, they will be willing and able to work independently as well as collaboratively within a team.
WHAT WE OFFER - Educational Incentive - An educational incentive of 2.5% for completion of an associate's degree, 3.5% for completion of a bachelor's degree, or 5% for completion of a graduate's degree.
- Bilingual Incentive - Incumbents may also be eligible for bilingual incentive depending upon operational needs and certification skills.
- Vacation Accrual - Minimum of fourteen (14) days earned per calendar year at time of hire up to twenty-six (26) days earned per calendar year maximum.
- Deferred Compensation - Eligible to participate in the County's 401(k) Shared Savings Plan and/or the Section 457 Plan. This position is eligible for up to a 2% match on your 401(k) contributions.
- Health Plans - Eligible for a flexible credit allowance for purchasing medical, dental, and/or vision insurance from a group of authorized plans.

Community Services Coordinator is represented by the Services Employees International Union (SEIU) and is not eligible for overtime compensation.
The eligible list established from this recruitment will be used to fill current and future Regular (including Temporary and Fixed Term), Intermittent and Extra Help vacancies for this position or similar positions only.
There is currently one (1) Fixed Term vacancy with an anticipated expiration date of 6/23/27. A Fixed Term position is a position designated by the Director-Human Resources or Board of Supervisors to which persons may be appointed for fixed terms not to exceed three (3) years. Fixed Term employees are eligible for the same benefits and compensation as Regular employees.

Examples Of DutiesDuties may include but are not limited to the following:
- Conducts patient outreach through multiple communication channels, including text, phone, emails, mail, and other platforms as appropriate;
- Assists with recruitment materials and engages with patients in clinic settings to recruit for study participation;
- Creates patient scripts and customized patient education material;
- Performs data analysis on study parameters;
- Plans, organizes, and coordinates assigned caseload and staff activities including case management, records management, and community outreach services and projects;
- Serves as liaison with representatives from state, County, and local agencies, school districts, courts and the community to identify and resolve problems/needs;
- Prepares a variety of reports, and correspondence;
- Keeps appropriate records, documenting patient encounters and progress notes consistent with agency requirements;
- Submits and maintains institutional review board applications;
- Collects data via surveys, focus groups, and interviews;
- Coordinates research operations, such as patient outreach and data collection;
- Assists with report preparation on study progress;
- Coordinates meetings to discuss study progress;
- Manages follow-up tasks between study meetings to ensure adherence to the study timeline;
- Communicates barriers to task completion and identifies alternative approaches;
- Meets productivity and quality goals set by agency; and
- Performs other related duties as required
Typical QualificationsThese are entrance requirements to the exam process and assure neither continuance in the process nor placement on an eligible list.
EDUCATION, TRAINING, and EXPERIENCE: Considerable experience in social work, community health, or education which has led to the acquisition of the required knowledge, skills, and abilities. The required knowledge, skills, and abilities can be obtained by:
- Four (4) years of experience in social work, community health, or education, including responsibility for program development and implementation
- At least one (1) year of the qualifying experience must be in community outreach and education
NECESSARY SPECIAL REQUIREMENTS: - Must be willing and able to work occasional weekends and evenings
- Bilingual in English and Spanish
- Intermediate proficiency in Microsoft Excel
- Excellent customer service skills
- Strong verbal and written communication skills
- Some experience leading and/or supervising a team
DESIRED: - Experience with Electronic Health Records such as Cerner
- Experience with research studies
KNOWLEDGE, SKILLS, and ABILITIES:
Thorough knowledge of: community and patient outreach practices; public health and/or social science topics; conducting patient-facing research in a community or safety-net setting; qualitative and basic quantitative analysis; community organization and resources; group and family dynamics; medical terminology; time management; needs and behavioral patterns of the population served; diagnostic, counseling, and referral techniques; federal and state laws and ordinances applicable to the program area; and specialized topics for outreach, education, and presentation.
Working ability to: effectively communicate complex programs and ideas to different audiences; perform statistical analysis; develop educational programs and evaluation plans; coordinate activities among various groups; provide services in a field-based environment; plan and organize work of others; conduct classroom training or educational workshops; communicate effectively orally and in writing; and exercise sound judgment in communications with patients and research staff.
Skill in: patient education; maintaining accurate records; and utilizing electronic health records.

Applicants must earn a score of seventy percent (70%) or higher to qualify for placement on the eligible list.
If there are three (3) or fewer qualified applicants, an oral exam will not be conducted. Instead, a score of seventy percent (70%) will be assigned to each application, and each applicant will be placed on the eligible list.
Applicants successfully completing the exam process may be placed on an eligible list for a period of one (1) year.
