Position Description:
DUTIES (This posting does not constitute all duties of the position):

Provide referral, education, advocacy, and follow-up support to residents of Licking County.

Assist individual seeking treatment for substance use disorder treatment or at risk of substance use disorder by connecting them with providers and arranging transportation.

Conduct follow-ups to address obstacles, ensure appointments are attended, and treatment plans are followed.

Facilitate transportation or assist with transportation for medical appointments

Provide referrals and assistance for other health issues

Participate in meetings and coalitions to identify resources

Collect and maintain data related to referrals and follow up activities

Distribute Narcan to the public via referrals and/or community events

Evenings and weekends will be required

Work may be conducted at LCHD’s Pataskala office

Maintain client confidentiality and privacy in all interactions.

Deliver culturally sensitive health education tailored to the client’s literacy level.
Brief Description:
Under general supervision from the Director of Personal and Community Health, the Public Health Community Navigator develops and implements support plans for individuals affected by substance use disorders or are at risk of developing a substance use disorder.
Job Prerequisites:
QUALIFICATIONS AND REQUIRED KNOWLEDGE:
Under general supervision from the Director of Personal and Community Health, the Public Health Community Navigator develops and implements support plans for individuals affected by substance use disorders or are at risk of developing a substance use disorder. This role collaborates with partnering agencies to create referral networks and improve access to treatment services. The Public Health Community Navigator helps reduce barriers to care and supports populations negatively impacted by social determinants of health. Job duties will require work to be completed in various parts of the county in conjunction with other Public Health Community Navigator.
Compensation:
SALARY: $50,000-$55,000
